Do you have a D-SNP plan or take care of a family member who has one? Learn more about how to make sure you get the care you or your loved one needs.
How does D-SNP work?
A Dual-Eligible Special Needs Plan, or D-SNP plan for short, is a Medicare Advantage plan for people who have both Medicare and Medicaid. Medicare is the federal health program that provides health care for older and some disabled people. And Medicaid is the state-run program that helps provide health care for people with low income.
If you have Medicare and Medicaid at the same time, a D-SNP plan might be a good option for you. Many D-SNP plans are available to members at no cost. These plans can offer great benefits for no cost because the Medicaid portion of the coverage covers any out of pocket costs for services covered by Original Medicare. If the Medicare portion of a member’s coverage doesn’t pay for something, then the Medicaid portion of the coverage can cover the difference.
What to do at the doctor’s office.
Once you have a D-SNP plan, how do you use it? It’s important to remember that even though your D-SNP plan combines Medicare and Medicaid into one plan, you still need to bring both ID cards. If you go to a doctor appointment, don’t forget to show both your Medicaid ID card and your D-SNP ID card from your health plan.
Use your extra benefits.
One of the best parts about D-SNP plans is that you can get extra care and benefits for no extra cost. Each plan is different, but for example—additional benefits covered by the Priority Health D-SNP plan include:
- $145 over the counter (OTC) allowance for personal health products.
- Care managers assigned to every member.
- Fitness benefits.
- Preventive dental coverage.
- Routine vision and hearing, including eyewear and hearing aids.
- And much more.
